Agreement for Therapy

Sasha Jenkin BA Registered Member MBACP (Accred)

It is my aim to offer you a safe space in which you can talk about what has been bothering you. Therapy is unlike other professional relationships you may have had. It requires active participation on your part and can bring up painful feelings.  If you feel able it can be helpful to bring up any difficult thoughts and emotions so that we can explore them together and understand them in the context of our relationship.

Timings

We will talk on Zoom weekly and sessions will last for 60 minutes. I will send you a link prior to our meeting, please click on this link at our agreed appointment time and I will meet you there online. If you are not available and we start later than booked we will still finish at the appointed time.

PLEASE ENSURE YOU ARE IN A PRIVATE SPACE WHERE YOU CAN TALK WITHOUT BEING OVERHEARD.

Cancellations

Please give at least 48 hours notice of a cancelled session. If I receive less than 48 hours notice, and we are unable to find an alternative time to meet within the same week, the fee is still payable.

Fees and Payment

My fee is £70.00 for an hour long session.  Please ensure payment is made before our appt by bank transfer.

Ethics

I abide by the Ethical Framework for The British Association for Counselling and Psychotherapy. A copy can be made available upon request.

Etiquette

You have the right to terminate the therapy at any time but where the work extends beyond 8 weeks I recommend that we have at least 2 weeks notice of ending to allow us a proper conclusion. A suitable ending period can be negotiated between us.

Please avoid taking affect altering drugs or alcohol before a session. If you arrive in what I believe to be an altered state that makes you psychologically unavailable the sessions will not take place and I will charge for the time.

Complaints

If you have a complaint about the therapy you can in the first instance take it up with myself either verbally or in writing. If there is no satisfactory conclusion you can bring the complaint to the BACP; please look at the Ask Kathleen service on their website www.bacp.co.uk

Confidentiality

The sessions are completely confidential except under the following circumstances:

  • I regularly meet with a clinical supervisor to discuss my work. This is standard practice and helps me to work to the best of my ability with you.  My supervisor is registered with the General Medical Council and bound by their confidentiality regulations.

  • If I believe you are at risk of harming yourself or others I reserve the right to break confidentiality in order to prevent harm. However, I would only do this in extreme circumstances and if possible would try to discuss it with you first before taking action.

  • If I am served a court order to give evidence.

  • If you disclose information regarding activities for which I have a statutory obligation to break confidentiality including acts of terrorism, drug trafficking & money laundering.*

Anti Discriminatory Practice
I am committed to an anti - discriminatory service. I strive to ensure that this practice is present in all our work together.

Data

I am committed to ensuring any personal information you provide and data that I collect is protected. Below I have outlined how any information will be handled.

  • I keep short hand-written notes on sessions which help me to reflect and practice more effectively. They are not identified by name and are stored in a folder in a locked cabinet. These are destroyed after 5 years of finishing our work together, in accordance with recommendations from my professional insurers.

  • Invoices will be stored on my personal laptop for a period of 7 years and then deleted.

  • Signed agreements and personal contact information are stored in a folder in another locked cupboard at a different location to the session notes. They will be destroyed after 5 years of finishing our work together.

  • Your first name and contact details will be included on a regularly updated list of current clients and kept in a sealed envelope so that in the event that I am unable to function adequately due to unforeseen circumstances such as an accident, sudden illness or death your details will be passed on to another accredited counsellor who will notify you.

  • You can request access to the personal information I hold about you. I will provide this within one month of your request, except in limited circumstances when I am not permitted to do so.

  • In the event of a data breach I will notify you.

  • You can request amendments to the personal data I hold if it is inaccurate or out of date. If you request I delete your personal information I will take reasonable steps to do so, unless it is required for legal, auditing or internal business purposes.

Before we proceed, I will be asking you to confirm you have read, understood and consent to the terms of this agreement, and a copy will be sent to you for your signature.

I will also need to know if you have any serious allergies or conditions I should know about.

Being based in the UK I am governed by English Law and, as such, if a claim is made against me it would have to go through the UK Courts.

*I can provide a document detailing these upon request.
